New and Returning Patient Resources
To help make your first visit to our practice as smooth as possible, please read the information on this page. You’ll find all the practical information you need, such as a map and directions to our office(s), hours we are open, payment policies and more. We also describe our first visit procedures. We strongly recommend printing off and filling our the New Patient Paperwork prior to your first visit.
Before You Arrive
Please arrive 15 minutes before your scheduled appointment if you are a new patient in order to complete the new patient registration paperwork. You can also print off and fill out the New Patient Paperwork by clicking the links above or in the dark blue area below.
Please bring the following items and information with you to your appointment:
Insurance Cards
Drivers License or government-issued photo ID
Medical Records and Reports (when able)
Hearing Test Results
Imaging Disc (MRI, CT Scan, X-ray)
Paper Referral, if required by your insurance company
Any Co-Pay required by your Insurance Plan
Upload or Fax Your Forms in
Ahead of Time
If you would like to fill out these forms online and send them to our office, you can do so by completing and submitting the online forms that are now available. These can be received securely. You can also fax them to 402-778-5216.
